GP CANINES SERVICE TERMS AND & RELEASE OF LIABILITY

NOTICE TO THE PET OWNER/GUARDIAN: PLEASE READ THIS DOCUMENT CAREFULLY. THIS AGREEMENT INCLUDES A RELEASE OF LIABILITY AND WAIVER OF LEGAL RIGHTS AND ELIMINATES YOUR RIGHT TO TAKE LEGAL ACTION AGAINST KATELYN LAKE (REFERRED TO IN THIS AGREEMENT AS “GP CANINES”), BY SIGNING THIS AGREEMENT YOU HAVE READ IT IN ITS ENTIREITY AND UNDERSTAND ITS EFFECT, POLICIES, PROCEDURES, PET RELEASE, WAIVER OF LIABILITY, ASSUMPTION OF RISKS AND INDEMNIFICATION AGREEMENTS.

  1. Health - I hereby represent that my pet has not had or knowingly came into contact with another animal with a contagious illness for 30 days prior to the service. I recognize that GP Canines is not a Veterinary service and they are NOT able to diagnose or provide medical advice for my animal. I represent that my pet has up to date vaccinations, I ACCEPT all risks associated with my pets health while under the care of GP Canines. I HEREBY AGREE TO INDEMNIFY GP CANINES, ITS OWNERS, EMPLOYEES, AND AFFILIATES AGAINST ANY CLAIMS MADE AGAINST GP CANINES AS A RESULT OF MY NEGLEGENCE TO INFORM GP CANINES OF ANY KNOWN MEDICAL CONDITIONS MY PET MAY HAVE.

    A) Illness- If you or anyone in your household are displaying flu/cold/Covid-19 symptoms we request that you to notify us to reschedule your session or wear a mask for the ENTIRE duration of your session. Cancellations due to illness may be made up to 24 hours before your scheduled service. Session will not be deducted or charged unless cancellations are made less than 24 hours before your session or in the event that is a no show. Exceptions to this policy may apply for extreme circumstances and will be left to the discretion of GP Canines.

  2. Transportation- I agree that in the event that my pet is being transported by GP Canines, its owners , employees or affiliates. I AGREE TO RELEASE GP CANINES, ITS OWNERS, EMPLOYEES, AND AFFILIATES OF ALL LIABILITY IN THE EVENT OF ACCIDENT OR INJURY TO MY ANIMAL DURING TRANSPORTATION.

  3. Pet Behavior - I agree to accept full financial responsibility for any and all damages caused by my pets behavior and acts while under the care of GP Canines. WHICH MAY INCLUDE PAYMENT OF DAMAGES AS A RESULT OF INJURY CAUSED BY MY PET TO HUMANS, OTHER ANIMALS AND/OR GP CANINES. ADDITIONALLY I AGREE TO ACCEPT SOLE FINANCIAL RESPONSIBILTY IF MY PET IS INJURED BY ANOTHER ANIMAL WHILE UNDER THE CARE OF GP CANINES, I HEREBY RELEASE GP CANINES, ITS OWNERS, EMPLOYEES AND AFFILIATES FROM ALL LIABILITY AND FINANCIAL RESPONSIBILITY RELATED TO THE INJURY.

  4. INCLEMENT WEATHER- We offer dog training year round, In the event that we are experiencing extreme weather (i.e. Torrential down pour, Snow storms, Thunderstorms, extreme wind, extreme heat or cold warnings) We will contact all clients to reschedule if it is deemed dangerous to train outdoors. If any clients or their pet cannot tolerate colder or warmer weather, and it is deemed dangerous for them. The client is responsible for contacting us no less than 24 hours prior to their scheduled service to reschedule. Rescheduling is dependent on our availability, wait times may vary . We recommend all clients bring proper attire for both themselves and their dog to their sessions.

  5. Photo and Video Release - I agree to allow GP Canines to use my pets name, and any photograph or videos taken while my Pet is in the care of GP Canines. I acknowledge that my pets photos and videos may be used at any time, for media, marketing, advertising, promotional materials, illustrations and personal use. GP Canines will keep your personal details, passcodes, and key/access codes confidential.

  6. Personal Property- I hereby understand that GP Canines will not be responsible for any lost, stolen or damaged personal property that belongs to me or my dog,

  7. Dog Training Services- 100% of the payment is due at the time of booking a service. We will not confirm dates or times until payment is received. All Training packages sessions purchased will expire 6 months from the service start date. All payments can be made via E-Transfer to Gpcanines@gmail.com

  8. Training Assessment - All New clients are required to purchase a Training assessment before starting a package. This assessment will allow us to get a thorough background on your pet and your training goals and provide us with the information to create your pets individual training plan. This assessment is charged at a rate of $50. Prices subject to change.

  9. Pricing and Availability - We reserve the right to change prices for our services without notice. We cannot guarantee availability - All available dates/times are first come first serve.

  10. Refunds - Training services are non refundable.

  11. Cancellations/Rescheduling- We require a minimum of 24 hours notice prior to the start of a scheduled service if wishing to reschedule or cancel a session. we understand life happens. We will not deduct or charge you for a session unless cancellations are made less than 24 hours before your service or a no show. Exceptions for extenuating circumstances may apply and will be up to GP Canine’s discretion.

  12. Duty to disclose- I represent that i have accurately disclosed and shall continue to disclose any and all medical conditions, personality concerns and behaviors that may affect, limit or inhibit my dogs ability to participate in the services provided by GP Canines. I acknowledge that it is my duty to ensure that I am being truthful, and that GP Canines is relying on the information I disclose to them to create a safe environment for both humans and animals during my scheduled services.

  13. INDEMNIFICATION AND WAIVER OF LEGAL RIGHTS- I RELEASE, INDEMNIFY, WAIVE, DISCHARGE AND AGREE TO HOLD GP CANINES, ITS OWNER’S, ITS EMPLOYEES AND AFFILIATES HARMLESS FOR ANY AND ALL DAMAGES, INJURIES, CLAIMS, LIABILITIES, LOSS, COSTS OR EXPENSES, LEGAL FEES, COURT FEES, CAUSES OF ACTION OR SUIT, WHATSOEVER IN LAW AND EQUITY, ARISING OUT OF OR RELATED TO THE SERVICES PROVIDED AT GP CANINES, ITS OWNERS, ITS EMPLOYEES AND AFFILIATES, INCLUDING WITHOUT LIMITATION SUCH AS: ANY INACCURACIES IN ANY STATEMENTS MADE BY MYSELF OR INFORMATION PROVIDED BY ME TO GP CANINES, ACTIONS OF MY PET, INCLUDING BUT NOT LIMITED TO DISTRUCTION OF PROPERTY, DOG BITES,INJURY AND TRANSMISSION OF DISEASE, AND ANY ACTION MADE BY MYSELF THAT IS IN BREECH OF THE TERMS OF THIS AGREEMENT

  14. SOLE AGREEMENT - This document represents the agreement made between GP Canines and the Owner/Guardian.

  15. AFFIRMATION: Each time I sign up for a service provided by GP Canines, I am re-affirming the terms of this agreement. I HAVE READ AND FULLY UNDERSTAND THE TERMS IN THIS AGREEMENT, AND UNDERSTAND THAT BY SIGNING THIS DOCUMENT I AM GIVING UP SUBSTANTIAL RIGHTS. I HAVE SIGNED THIS DOCUMENT FREELY AND VOLUNTARILY WITHOUT COERCION, OR ASSURANCE, AND INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY FROM GP CANINES, ITS OWNERS,EMPLOYEES AND AFFILIATES TO THE GREATEST EXTENT PERMITTED BY LAW. I FURTHER AGREE THAT IF ANY PORTION OF THIS AGREEMENT IS HELD TO BE UNENFORCEABLE OR INVALID, THE REMAINDER OF THIS AGREEMENT WILL REMAIN IN FULL AFFECT.

GP CANINES TRAINING TERMS AND AGREEMENTS

At GP Canines we do our best to create a safe environment for all of our clients. The training methods we use are safe and reliable. Owners must acknowledge that dogs are still animals and can act unpredictably. GP Canines will not be responsible for damages to property or injury caused by your pet.

Pet Owners must understand that any recommendations for Training tools or Training methods provided by GP Canines does not guarantee a dogs performance or owner satisfaction. All Family members in the household must adhere to and not waiver in any training methods provided by GP Canines. This can result unsatisfactory results and slower progression in your dogs training.

The Pet Owner must understand and agrees with the use of any Training tools and Techniques that may be used by GP Canines to aid in your dogs training. These can include but are not limited to the following: Prong collars, Flat collars, Slip leashes, Long lines, Harnesses, Head collars. Methods and tools will be discussed with the owner prior to being used on the dog.
